Code 4 - S3-E4

Audio problem: When detective Bryant gets rough while interrogating the suspect of the shooting he tells him to sit down. When we cut to a different angle we hear him repeat the last few words of his sentence.

Show generally

Revealing mistake: In the episode "Identity," in the scene where Cooper takes the homeless man's photo on his iPhone, if you look to the bottom left of his iPhone's screen where it shows pictures that you have already taken, you can see that the homeless man's photo has already been taken, indicating that perhaps this scene was shot a few times, and the photos taken during those scenes were never deleted.

Wednesday - S4-E1

Continuity mistake: Near the end of the episode when a gunman enters the precinct house and starts shooting, Sammy dives for cover and begins looking for his gun in his duffle bag - twice.

Graduation Day - S3-E10

Continuity mistake: In the opening scenes Adams is in bed with her patrolman boyfriend. She is on his left and says we need to talk. In the next camera angle Adams is now on the boyfriend's right side.

Failure Drill - S3-E9

Continuity mistake: Adams and Ochoa are in a factory trying to find the shooter with a machine gun. Adams is significantly ahead of Ochoa in the stairwell, yet, when they enter the floor Ochoa is in the lead.

Trivia: In 2014, the series creators and network were sued for emotional distress by the family of a man named Andy Abarca. Abarca had been killed in a shooting, and actual photos from his autopsy were used in the opening credits montage of the show, evidently without any permission from his family. His mother and sister just happened to stumble across the show on one night in 2013 and were greeted with grisly images of their deceased relative. It was evidently allowed as autopsy photos are considered public documents. As of 2018, there have been no updates on the case.
