Other mistake: While Roy is showing Brice the squad, after Brice annoyingly locks Squad 51's side bay door, he asks, "How about the OB box?" but the subtitles read, "How about the OV box?" Then a minute later Brice says, "Let's look at the OB box," and again the subtitles read "OV box."

Other mistake: When the ambulance arrives and they're loading Johnny onto the ambulance, Captain Stanley says, "Okay, Chet, take the squad..." but the subtitles get both characters wrong, it reads that a "Paramedic" says, "Okay, Chip, take the squad..." There is no Chip.

Continuity mistake: At the nurses' station, after Roy tells Brice that Brightweiser will be okay the two of them walk to receiving carrying their gear, with Roy holding the Biophone and defibrillator, but as they head through the doors both the Biophone and defibrillator have each turned around in Roy's hands, and now face the opposite way.

Other mistake: When Early and Morton are examining Johnny, Early points out Johnny's torso as he says, "Mike, you see this bruise?" but the subtitles read, "Frank, you see this bruise?" A couple of minutes later the subtitles get Mike's name right.

Continuity mistake: When Johnny is being examined by Dr. Early and Dr. Morton, Roy is in the treatment room with them, and he's not wearing a watch, but when Roy steps into the hallway and speaks to Chet he's suddenly wearing a watch.

Other mistake: When Early and Morton are examining Johnny, Early tells the nurse, "Type and cross" (the blood vials she's holding), but the weird subtitles read, "Cut and cross."

Continuity mistake: When Early walks into the staff lounge after Johnny's surgery, and joins Roy at the table, Early's holding the cup of coffee with his right hand in the shot facing Roy, but in the shot facing Early he's holding the cup with his left hand.

Continuity mistake: While Early is examining Johnny in the ER, telling Roy about symptoms for ruptured liver or spleen, in the closeup Roy's holding up the IV bag with his right hand, but next wide shot it's held up in Roy's left hand.

Breakdown - S6-E15

Charlie: I think you guys are playing games with me, and I don't like it.
Roy: Now, wait a minute. We're not playing games. We don't play games with the equipment...
Captain Stanley: Wait, hold on. C'mon everybody, look we're all on the same side here, aren't we?
Charlie: I wonder.
Captain Stanley: Charlie, why can't we put the squad in the shop and have us a reserve vehicle here?
Charlie: No, no, Hank. Not until I'm 100% sure that I can't fix it. If there's anything wrong with it. This coffee stinks. [Leaves the room.]
Captain Stanley: You guys playing some kind of joke on him?
John: We didn't make the coffee!
Captain Stanley: Not the coffee, you twit, the squad.
Roy: Cap, there's something wrong with that squad out there.
Captain Stanley: [sigh.] All right, if you say so... I made the coffee.

Surprise - S4-E6

Question: When the guys are hanging off the roof does anyone else see a white wire attached to the sign?

Answer: Yes, there's a thin cable secured to the sign's bottom mounting bracket which leads upward to the crane arm, and can be seen in quite a few shots at various angles. It's reasonable to presume that this cable which is secured to the crane was part of the sign's mounting process to the two wall brackets before the winds caused the accident, the premise of this rescue. For this reason it really can't be noted as "stunt wires being visible" or something like that.
