Visible crew/equipment: When Johnny and Roy are on the soap opera set treating the 'doctor', the outlines of the lav mic transmitters are visible on the front, left sides of Johnny's and Roy's shirts, above their belts, and the outline of Jody's mic transmitter is visible on the back of her brown sweater.

Visible crew/equipment: When Johnny is reading out loud from a transcript, while he and Roy head to the tape review meeting in the staff lounge, just after they turn the corner, the three tape marks are visible on the floor. One is for Johnny near the door, and two are for Roy - the first mark for where he originally stands and the second one for when he walks back over to Johnny.

Visible crew/equipment: When Roy, Johnny, and Morton leave the staff lounge talking about Brice, they stop at the nurses’ station, and when Johnny and Roy walk away, the two actors’ tape marks are visible where Morton is still standing and where Roy had just been standing. Additionally, the outlines of the lavalier mic transmitters are visible on the left sides of Roy's and Johnny's shirts, particularly when they stop at the fountain.

Revealing mistake: When Johnny, Roy, Vince, and the wounded officer are hiding between the bakery truck and car, before the multiple bullet holes actually appear on the side of the truck, those areas where the holes will show up have off-color markings, prepared for the stunt.

Breakdown - S6-E15

Visible crew/equipment: When the guys are on the tower ride during the rescue, the shadow of the chopper (and its rotating rotor blades) doing the aerial filming becomes visible, when the ride reaches the bottom. Then the shadow of the entire chopper moves in directly in front of the ambulance, as they load the ambulance.

Visible crew/equipment: At the station, when Johnny and Roy talk about the stewardess and then while Chet is mopping the floor, the bulging outline of the mic transmitter is visible on the front, left side of Roy's shirt, above his belt.

Visible crew/equipment: At the start, when Johnny is telling the stewardess about the underwater rescue and in following shots, the outline of the lavalier mic wire and the dark strap (that holds the transmitter) around Johnny's waist are visible on the back of his shirt, and the mic transmitter is visible on the front, left side of his shirt, above his belt.
