The Boat - S6-E19

Continuity mistake: When Roy and Johnny are on the Coast Guard helicopter, responding to the boat wreck with injuries off Catalina, there are a few interior shots of them that are flipped, while they're looking out the windows. The paramedic patches are on their left sleeves, but in the flipped shots the paramedic patches are on their right sleeves.

Visible crew/equipment: When Roy and Johnny respond to the boat wreck with injuries off Catalina, they pull up to the Coast Guard Air Station, and when Roy joins Johnny on the other side of Squad 51, the moving reflections of two crew members are visible on the back of the squad.

905-Wild - S4-E22

Revealing mistake: When the Bengal tiger is on the roof, just after the officer warns, "If that cat tries to leave this roof, he's dead before he hits the street," there is a closeup of the tiger and the empty landing below the billboard. A few shots later, after Les steps on the glass, there are pieces of meat scattered on the landing below the billboard, so the trained tiger will jump onto it.

Revealing mistake: At the start of the episode, we see Squad 51 speeding toward their destination, and when the camera cuts to the woman in front of the market, she's waving her arms to the squad somewhere in the distance. But notice that while Rosa is looking to her left and waving, we can see Squad 51's reflection in the window (with the liquor bottles), as they are parked right there, just off-screen waiting for their cue to "speedily" pull up.

Equipment - S5-E4

Deliberate mistake: After Johnny's and Captain Stone's efforts, when the construction site's cardiac arrest is wheeled into Rampart, the problem is twofold; First, we hear Roy's voice saying, "Full arrest, number four," even though Roy is not actually on this run (that line is from S4xE21 "Back-Up", when they wheel the cardiac arrest into treatment 1, after defibbing him atop Engine 51). And second, the attendant pulling the stretcher changes, and the EKG monitor lying on the patient is the old blue Datascope 850 from earlier seasons (this shot is from S4xE7 "Daisy's Pick", the hypothermic victim - note his wrapped foot!), whereas at the construction site and when he's wheeled into the treatment room, the EKG monitor is the newer white Datascope M/D2.

Continuity mistake: During the warehouse fire, Johnny removes his turnout when Captain Stanley calls for the ambulance, then when the ambulance arrives John's wearing the turnout and one burn victim has the sheet over him, but then the turnout and sheet vanish, and as the ambulance attendants bring the stretcher, Johnny places the sheet on the victim, and in the overhead shot as Cap is told about Willy, Johnny's wearing his turnout again. The Biophone also keeps jumping from being beside Squad 51 to being between the victims.

Deliberate mistake: After the fireworks begin exploding at the warehouse fire, when Captain Stanley and Roy are helping Willy out, they are knocked to the ground by an explosion, and in two of the following exterior shots the large sign that reads "Kiddytime Toy Co" is visible. This shot is from season 4, episode "The Screenwriter", when the guys respond to the fire at the toy factory that also made fireworks.

Continuity mistake: When Johnny is doing overtime with Engine 8, dispatch sends the two guys of Squad 8 on an unknown type rescue and they leave Station 8, but when Johnny and Captain Stone walk back into the kitchen, Squad 8 is still parked in the apparatus bay.

Continuity mistake: When Johnny is being examined by Dr. Early and Dr. Morton, Roy is in the treatment room with them, and he's not wearing a watch, but when Roy steps into the hallway and speaks to Chet he's suddenly wearing a watch.

Factual error: Roy uses the defibrillator paddles on the electrocuted man twice, then after each defib Roy lifts the paddles off the patient's chest and they look at the EKG monitor, which shows flatlining the first time and a heartbeat the second time. Problem is, it's impossible for the EKG monitor to show anything at all. Either the defib paddles must be in contact with patient's body for the “quick-look” to determine the rhythm, which they were not, or the ECG electrode discs must be on his chest connecting him to the EKG monitor, and they were not.

Other mistake: When Engine 51 pulls up to the hotel with the injured man dangling from the hotel sign, Captain Stanley addresses Chet and says "Chet, Roy and John'll need a line," but the subtitles read, "See that? Roy and Dan'll need a line."

Other mistake: When the tones drop and Station 51 is dispatched to a "man injured", there is a closeup of the guys running from the kitchen into the apparatus bay, heading for the squad and engine, and none of them are actually 51's guys.

Other mistake: During the fire at the chemical plant, when Captain Stanley and Roy find the missing man and carry him out, the subtitles read that Johnny says "All right, grab his feet," when in fact it's Captain Stanley who says that - Johnny's not even in the same room with them.

Continuity mistake: During the fire at the chemical plant, the distinctively marked SCBA tanks that Captain Stanley, Johnny and Roy are wearing, when they're searching for the missing man, change when they're inside and then finally exit. Note the different stickers, or lack of stickers, on their tanks.

I'll Fix It - S4-E2

Continuity mistake: When Station 51 is at the scene of the oil well eruption, underneath a house which was built atop an abandoned oil field, just after Johnny tells Roy that he thinks the whole foundation is slipping, it cuts to a shot of the front porch as one of the porch columns (and part of another) breaks and falls. However, when the guys finally free the trapped man and carry him out, all the columns are upright once again.

Trivia: Blink and you'll miss it. At Rampart, when Art walks away from the nurses' station, much to Dixie's relief, he heads toward the waiting room and then Roy and Johnny follow him. Pay attention to the guys in the background as they're nearing the waiting room, Johnny quickly pokes the nurse in her side, as he passes her. (00:13:05)

Visible crew/equipment: During the structure fire, when Roy, Johnny and the security guard are in the upstairs room and Roy closes the doors, just as they start to walk toward the window, the moving shadow of the cameraman and camera are visible on the wall. The shadow is not moving in sync with the guys so we know it's not theirs.
