Continuity mistake: When Dixie opens the door to Brackett's office, there's a pile of white papers on the desk in front of Brackett, but when he walks out of the office, in the view from the hallway, the desk lamp has moved and there are no papers where there were in the previous shot.

Visible crew/equipment: After Johnny tells Roy not to phone Joanne about the inheritance, when Chet walks into the kitchen announcing that it'll cost $2.25, the small black tape mark is visible on the floor where Johnny and Roy were standing.

Other mistake: After Mr. Winthrop is brought to the ER, there is a closeup of his EKG monitor as it's picking up multiple PVCs, but when Brackett asks him about the tension in his life, the monitor seen behind him is showing a flat line. This happens once again after Roy and John leave the treatment room, just before they move him to CCU.

Other mistake: After Roy and Johnny get back to the station, when Johnny takes the container of milk out of the fridge, we can see the black tape around it covering the brand name. There's another container in the fridge with red tape covering its brand name as well.

Visible crew/equipment: When Station 51 is dispatched to the boat explosion, just after Captain Stanley responds, "KMG365," as he walks past the camera to the engine, we can see the open top of the set at the rear corner of the apparatus bay.

Visible crew/equipment: After Roy breaks the window with the chair, the guys place the ladder against the window and Marco starts to climb up. In the next interior shot, just as Roy and Johnny lift the security guard, right outside the window we can see the corner of something else (platform-like), besides the top of the ladder.

Continuity mistake: At the apartment complex fire, after Truck 82 sets its outriggers, Engine 28 is right behind them with their supply line and duals laid out, but quite a few shots later Engine 28 is just pulling up behind 82, even though they were already there all set up.

Visible crew/equipment: At the apartment complex fire, when Truck 82 raises its ladder, we can see the second floor balcony behind the rig, and there is a crew member wearing a long sleeved blue shirt and white hat, standing in front of the corner apartment, where there should be smoke pouring out, but there is none. The reflection of the cameraman/crew member wearing a white hat also becomes visible in Truck 82's wing mirror.

Revealing mistake: During the fire at the apartment complex, there are strips of black material nailed to the tops of windows where the FX fire shoots out from below, and the doorway Roy passes when he's pulling on the hose has a fake ceiling and wall nailed directly into the door jamb, to control the strip of FX fire at the side.

Revealing mistake: At the apartment complex fire, just as Chet climbs the ladder and Marco holds the top of the ladder on the second floor balcony, we see the plumes of FX smoke spurting from the side inside the doorway behind them, and even see something quickly fanning the smoke.

Continuity mistake: During the fire at the apartment complex, when John and Roy go into the apartment to rescue the woman, they throw a coat over her, but then the explosion occurs knocking John and the woman unconscious. When Roy puts her over his shoulder and starts walking out the coat is not over her, but when he gets to the balcony she's covered by the coat, yet when the Pasadena fireman goes back for John, that coat is still lying on the floor.

Visible crew/equipment: At the site of the vehicle accident, the outlines of the lavalier mic and wire are visible on the back of Roy's shirt, particularly when he's with Eddie as his father's being taken in Copter 10 to Rampart.

Visible crew/equipment: After Engine 51 and Copter 10 are dispatched to the car accident, when Copter 10 takes off, the moving shadow of the filming helicopter is visible near the shadow of Copter 10.

Visible crew/equipment: After landing at the base camp, when Brackett and the others walk away from Copter 10, we get a bird's-eye view of the base camp, and can see the moving shadow of the filming helicopter at the lower right corner of the screen.

Continuity mistake: When Station 51 is dispatched to a structure fire (with the two girls trapped upstairs) the address given is 1911 Harper Ave, but when they reach the house with the fire, directly across the street that house number is 1637.

Visible crew/equipment: When they wheel Dianne (the OD) into the ER it's one long continuous shot from receiving to treatment 1, shot from the POV of the patient lying on the stretcher. As she's wheeled into treatment 1, the camera faces Johnny so we can see up the hallway behind him, and the entire admitting desk and waiting area are all blacked out, and there are two men, one of which (possibly the director) is sitting on a chair facing up the hallway where Johnny is and the scene is taking place.

On Camera - S5-E12

Continuity mistake: When the boy who fell over the cliff is brought into the ER, Dix tells them to bring him to treatment 3 and we see them at treatment 3's door, which is on the same side of the hallway as treatment 4, with the red line closest to it (hall has red, green and yellow lines on the floor which run the length of the hall) However, in the shot from inside the treatment room, they're not in treatment 3, but are actually in treatment 1. Note that we see treatment 4 directly across the hall, which means they're in treatment 1, and also see the yellow line is closest to the door which means it's on the opposite side of the hallway.

Visible crew/equipment: When Squad 51 is heading to 1511 West Hill, responding to the attempted suicide, in the closeup of Roy and Johnny en route, we can see the reflection of Squad 51 as they're driving past houses/stores on Johnny's side. Just about as Roy says "It should be it over there," in the reflection we can see two crew members wearing white hats riding on the squad - one is riding on the top of the squad, and the other one is riding on the back.

Other mistake: When John and Roy get back to the station, they head to the kitchen and John takes the hot pot of stew out of the oven with potholders, and when he quickly tries to lift the lid without the potholder, his pain makes it clear that it's burning hot, so he uses the mitt to lift the lid. There's a long metal serving spoon that was left in the stew, and John just lifts it up and starts serving the stew with no problem, even though that spoon should have been too hot to even touch.
