Rules of Order - S6-E6

Visible crew/equipment: When Station 51 is dispatched to an unknown type rescue, which turns out to be two people stuck on the side of a building, in the first shot of them the aerial camera tilts down and pans right to left, and as it does the reflection of the crane (supporting the camera) is visible on the building's glass siding, and the crane's outboard legs are also visible as the camera follows the squad into the lot.

Rules of Order - S6-E6

Visible crew/equipment: After Early and Brackett do the tracheotomy, when Dix leaves treatment 4 and walks up the hallway, the outline of the mic transmitter and wire are visible on the back of her uniform top, and when she sees Johnny in the hallway, the outline of his mic transmitter is visible on the front of his shirt over his belt.

Quicker Than the Eye - S4-E8

Visible crew/equipment: After Chet's disappearing half-dollar trick, when Squad 51 is dispatched to the gunshot wound, as Roy and Johnny get into the squad numerous set lights are reflected on the squad's chrome spotlight.

Visible crew/equipment: After Johnny tries to take Chet's picture beside the map, when Captain Stanley walks from the office to the back of Squad 51, the reflection of crew/equipment are visible on the back of the squad.

Camera Bug - S4-E10

Revealing mistake: When they wheel the stretcher with the unconscious teenager from the school fire into treatment 3, the stretcher is pushed up against the ER's gurney, and when the teen's right arm gets caught between the stretcher and gurney, the supposedly unconscious teen moves his arm up out of the way. Then he lifts his left hand when the attendant removes the blanket.

Camera Bug - S4-E10

Trivia: After Brackett asks Dix if she'd like to go to lunch, it cuts to a shot of Johnny balancing himself above Roy, between the engine and squad, while snapping Roy's picture, and when Johnny jumps down right beside Roy's head, Roy quickly flinches back and Johnny giggles an apology. Later, after Johnny snaps Captain Stanley's picture, Cap tells John, "You blinded me you twit," and when Johnny removes the camera from around his neck he smacks the camera right into Roy's face, and Johnny gently pats Roy's head. Good unscripted moments between Roy and John. (00:21:45 - 00:41:10)

The Firehouse Four - S4-E11

Continuity mistake: While Johnny tries to reach the man who fell over the cliff, when the bicycle falls to the bottom of the canyon both tires break free from the frame, but when the officer retrieves the bicycle, only one of the tires is off, the other is undamaged and on the frame.

Welcome to Santa Rosa County - S6-E10

Visible crew/equipment: After Roy and John eat lunch with the sheriff and decide to head to the lake after lunch, when they both get into the squad car, we can see the reflection of the cameraman, who's wearing jeans and a dark shirt, in the wing mirror, as he walks closer to the car and bends down. We can also see the legs of another crew member standing behind him.

Visible crew/equipment: When Sheriff Bittner, John, and Roy are driving to the lake after eating lunch together, the bar for the mounted camera is visible on the driver's door, which is only seen in the interior shots of the car.

Revealing mistake: After rescuing the burn victim from the boat's explosion, Johnny and Roy are at the back of the camper with the victim, while the victim's wife drives the camper to the clinic, but when they arrive at the clinic the camper hasn't even stopped yet, and we can see the woman's legs at the side of the camper, waiting for her cue to come rushing to the camper's rear door when her husband is brought out.

Visible crew/equipment: After Sheriff Bittner breaks the door's window to get into the clinic, when they open the door just before they walk inside, the camera's tripod, the corner of the reflector screen, and a crew member wearing a blue/white cap are all visible through the doorway by the porch area.

Paper Work - S6-E11

Revealing mistake: At the start of the episode, when the camera zooms in on the words 'Los Angeles County Fire Station 51' which are attached to the side of the building, we can see that this shot is actually played in reverse - notice the cars/trucks behind the station are driving backwards. Additionally, note that the real 127's rig is seen in the driveway to the right. Also interesting to notice are the upper/lower metal bars which are attached to the bricks, where we can see the bars are much longer after 'Station 51', with a space for something missing, due to the fact that is was made to hold '127' on it.

Visible crew/equipment: When the woman drives her car up to Station 86 with the two injured passengers, the outline of mic transmitter is visible on the back of her sweater, when she gets out of the car.

Visible crew/equipment: After the rescue on the draw bridge, when Squad 51 gets back to the station Roy goes to the backyard and sits in the sports car, and in this shot just as we hear Johnny's footsteps, on the Land Rover's window we can see the reflection of a crew member who is moving backwards.

Other mistake: When Station 51 is dispatched to 'man trapped in vehicle', we can see the real Station 127's vehicle is still parked inside the apparatus bay, near the rear bay door.

Quicker Than the Eye - S4-E8

Continuity mistake: When John and Roy walk into the house with the pregnant gunshot victim, only John's wearing a helmet and he places it on the floor near the victim's husband, but in all of John's closeups (from the unusual POV looking up at him) his helmet is on the counter. Additionally, there are blue glasses upside-down on the side of the dish drainer, but in those closeups of John the glasses have vanished, and the glasses vanish again when Roy helps the husband get on his feet. Also notice that there's now a blue glass in the sink.

Visible crew/equipment: In the locker room, after Chet tells Johnny that he knew all about the practical joke, when they walk to the door together there's a quick glimpse of the cameraman's reflection on the door's glass, before John tells Chet about the fictitious five dollar bill down by Mike's locker.

Continuity mistake: There are only two ambulance attendants who take the pregnant gunshot victim to the vehicle, but en route we see two attendants in the front seat - driver and passenger, and a third attendant is in the back with John and the victim. Then when they reach Rampart, the attendant that was in the back with John has vanished and he's the one who opens the back door from the outside. As an aside, in the ambulance, when John talks to Rampart about fetal heart tones, we see the attendant is actually mucking around with the IV drip.

Visible crew/equipment: When Roy and John get back to the station, and Marco walks up to the squad to talk to the guys about the practical joke they're going to play on Chet, on the squad's spotlight we can see the reflection of the camera, which is in front of the map.

Trivia: During the scene with the pregnant gunshot victim, the husband is played by Mark Spitz, who won 7 golds at the infamous '72 summer Olympics. Notice the logo on the red polo shirt that he's wearing is the USA Olympic shield logo.
