Continuity mistake: There's much too abrupt a transition between Reese falling into the water and his body suddenly being back on the dock. As there's no time lapse implied at all, he seems to reappear on shore by magic. At least a brief shot of the body being fished out of the water might have helped. (00:49:40)

Continuity mistake: "Stop When Red Lights Flash": In the opening scenes, Weston's rear view mirror is present in exterior shots of the car, but missing in all the interior shots. (00:02:00)

Continuity mistake: "The Time of the Wolf": Robin slays his evil twin and leaves him on the grass near the standing stones. The body then changes positions without anyone touching it. When Robin leaves, its head is pointing to screen-right. But when Marion arrives, Evil Robin has done an about-face: his corpse is now pointing to the left. (01:21:00)

Revealing mistake: McGarrett's office is on the 2nd floor. Yet the photo backdrops outside his windows in this episode show us a ground-level view of grass, streets and a building. This boo-boo vanishes in subsequent episodes, replaced with the more likely views of balconies, palm trees and blue sky. (00:08:00)

Visible crew/equipment: The tailgate of Lloyd Chandler's station wagon, and the door of Gerard's car a few minutes later both capture reflections of studio lights and the camera crew. (00:04:50 - 00:07:10)

The Judgment (2) - S4-E29

Revealing mistake: The last scenes of the grand finale/climax occur in an abandoned amusement park that's supposed to be in inland Indiana. But several shots with the Pacific Ocean "lurking" in the background (despite the cameraman's best efforts to hide or avoid it), as well as palm trees, and ocean sounds intruding on the soundtrack, reveal that the actual shooting location was in Southern California. Continuity mistake: "Stop When Red Lights Flash": The invisible Weston spies on A.J. and Charlie as they open the safe, observes the combination, and then waits until they leave to open the safe himself. But the combination he dials isn't the same one they just used. (00:20:40)

Revealing mistake: "Stop When Red Lights Flash": In the cell with Kate, Weston puts on his mask, making only his head visible. But the blue-screen matte used for this special effect is, weirdly, showing through his blue eyes. (00:23:40)

Continuity mistake: Kimble tells Hobart there's no new evidence that could get him another trial. But maybe Dr. Kimble is suffering from amnesia. In the past 3 1/2 seasons, he's uncovered no fewer than four potential witnesses who could provide new evidence in a second trial. (00:22:35)

Plot hole: "Sight Unseen": Wells has the key to Lori's shackles. Weston, who knows this, knocks Wells out, but instead of taking his keys, uses a completely unnecessary (and considerably more difficult) set of bolt cutters to break the leg cuffs. (00:32:30)

Continuity mistake: Mundy lines up six golf balls and hits one, leaving five - until the shot reverses angles, anyway. Then there are only three golf balls left on the ground. (00:40:00)
