Continuity mistake: McGarrett strong-arms Tommy-Tommy, the Hawaiian fence, on the beach, and the guy gets sand all over his face during their scuffle. When McGarrett pulls him to his feet, the sand is still there. But in the next shot, all but a small patch near his temple has disappeared. (00:32:40)

The Mother Muffin Affair - S1-E3

Revealing mistake: The climactic chase occurs in an amusement park, supposedly in Brighton, England. But several things give away the real filming location, among them an ad for 15c (not 15p) snow cones, two very American-style phone booths, and best of all, a pole proudly sporting both a U.S. flag and a California state flag, visible in the background behind the phone booths. To Catch a Roaring Lion - S2-E12

Continuity mistake: The professor translates the two Chinese symbols from Mundy's brass rubbing. Then he's shown a Chinese business card, and states that the 12 symbols on the card say the same thing. But not one of the symbols on the card matches the first two from the paper. (00:24:25)

Audio problem: When Katrina walks out of the building at the end and crosses the street to Dover's car, the sound of her boots on the pavement doesn't match the actual movement of her feet at all. (00:45:10)

Deliberate mistake: The bad guys chase Mark through a movie studio backlot past a London bus with a large red "Out of Service" sign on its side. During the ensuing shoot-out scene, the film is reversed for some reason, and the sign on the bus is suddenly backwards. (00:24:20)

Continuity mistake: McGarrett gets out of his car wearing a gray suit, and walks into Dennisen's office wearing a blue one. (00:20:15)

Visible crew/equipment: In the teaser, the departing Thrush limo provides the viewer with a reflection of both the camera and the lined-up film crew. (00:02:15)

Continuity mistake: When the Bannister gang begins shooting at the house, the ladies abandon their breakfast to run and hide upstairs. There's a pitcher half full of milk on the table. But a few shots later, when one of the Bannisters bursts into the kitchen, the milk pitcher is empty, and the dishes are in different positions on the table. (01:23:55)

Samurai - S1-E6

Factual error: Throughout this episode, the Japanese assassins are inaccurately referred to as "the Bushido." They should, as the episode title implies, be called Samurai. Bushido was the revered code by which Samurai warriors lived, but was never an appellation for them. (00:17:30)

Continuity mistake: When Avon & Co. barnstorm the ruined lab, a large piece of plastic debris on the floor is there when they enter, but then disappears between takes. Avon actually slips on it coming in, and does a hilariously un-macho little balancing dance. To quote (and agree with) actor Paul Darrow: "I can't believe they left that in!" (00:45:00)

Time Squad - S1-E4

Visible crew/equipment: An electrical set cable, poorly concealed between the seat cushions, tangles around Jenna's foot as she's getting up from the teleport console, and causes her to trip. (00:37:00)

A Sour Note - S2-E2

Factual error: Gino commits a murder using a six-shooter with a silencer attached, and his shot doesn't make any noise. But it should have. Though it's a long-standing Hollywood cliché, a revolver with a silencer wouldn't be silent at all. Unlike automatic weapons, revolvers are open-chambered, which allows sound to escape. They can't be muffled with a silencer. (00:34:00)

Continuity mistake: During his scuffle with Gino, Mundy's hair is messed up, then neatly combs itself, then is messy again, and so on several times between takes. (00:45:45)

Continuity mistake: At the party, Mundy is handed a cocktail that's blue in color. Next shot, when he starts to drink it, the cocktail has changed colors - from blue to amber. (00:07:05)

Factual error: McGarrett places the kidnappers' ransom tape on the reel-to-reel recorder, and before the tape is threaded or the machine properly turned on, the message immediately starts to play. (00:13:35)

Revealing mistake: When he sits on the floor in front of Servalan, Jarrier's earring falls off. He fumbles at it briefly, and then the actor breaks character for just a moment, glancing nervously at the camera. When it's apparent there won't be a retake, he goes on with his lines. (00:16:10)

The Lay of the Land - S1-E16

Revealing mistake: Lori Chan picks Mundy up at the airport and proceeds to speed through town, spinning the steering wheel left, right and left again repeatedly. But the rear-screen projection behind them is showing footage from a car moving steadily in a straight line. (00:07:45)
