Quantum Leap (1989)

Lee Harvey Oswald (1) - October 5, 1957 - November 22, 1963 - S5-E1

Continuity mistake: In the Japanese bar, when Sam takes a drink from the beer bottle, the label is facing him. When Oswald (who is Sam's reflection in the mirror) takes the same drink, the label has rotated several degrees and is facing outward, away from him. (00:28:15)

Monk (2002)

Mr. Monk and the Leper - S5-E10

Continuity mistake: In the final scene, the pillows on Natalie's couch keep shifting positions between shots, from straight to crooked and back again. (00:40:30)

Continuity mistake: When Thing is using sign language to tell Gomez that Morticia is in danger, there's a blue coffee mug sitting on the table (to Thing's immediate right) that vanishes every time the camera angle reverses. (01:25:00)

Beyond a Treasonable Doubt - S3-E23

Audio problem: When Mundy escapes, Devon shouts, "Stop him!" But his lips aren't moving when we hear the words shouted, and when his lips do mouth the words several seconds later, there's no sound. (00:10:00)

Jean G

Beyond a Treasonable Doubt - S3-E23

Revealing mistake: Somehow you'd think that a gadget-happy government spy agency would be capable of replacing a dead battery. The clock in the SIA HQ board room remains at 10:10 throughout the entire 5-minute interrogation scene with Mundy. (00:07:00)

Situation Red - S3-E19

Deliberate mistake: We see the Air Force plane's bomb bay doors come open. But in the next shot of the plane banking, the doors are instantly closed again. (00:45:20)

The High and the Deadly Affair - S1-E28

Continuity mistake: Merek's bird is sitting on his shoulder facing backward. In the very next shot, as he stands, the bird is suddenly facing forward. When he starts to sit down, the bird is still forward, but in the next shot, a close-up of him sitting, birdie is facing to the rear again. (00:07:50)

Show generally

Continuity mistake: "The Preying Mantis": The Black Beauty leaves Britt Reid's garage in full daylight. In the very next shot, as the car pulls out into the street, it's completely dark. (00:03:35)

Fortune City - S3-E18

Revealing mistake: The saloon's upper level railing looks solid until just before the bad guy falls through it, when the cuts in the break-away section are prominently visible. (00:26:05)

Show generally

Visible crew/equipment: "The Preying Mantis": The Green Hornet zaps the bad guy's rifle with his stinger ray. When the gun bursts into flames, the wire leading to the explosive special effects charge is visible trailing off to the right of the shot. (00:17:30)

The High and the Deadly Affair - S1-E28

Continuity mistake: The chocolate eclair is cut open to reveal the mini-bomb planted inside. But when April picks the eclair up, it's completely intact again. (00:43:30)

Jean G

Fortune City - S3-E18

Continuity mistake: Mona draws a heart in the dust on the bar and writes "Al + Mona" inside it. When we see a close-up it, however, the L is suddenly much larger than before, and the name "Mona" is written in smaller letters. (00:04:00)

Jean G

To Sing a Song of Murder - S3-E20

Continuity mistake: At the airport, Mundy asks Marilyn what's wrong and places his hands on her shoulders. Then the shot cuts to a reverse angle, and his hands are resting on top of the car door instead. (00:07:50)

Hawaii Five-O (1968)

King of the Hill - S1-E15

Continuity mistake: Kealoha puts his gun back in its shoulder holster before walking a few feet across the parking lot to meet McGarrett. When we cut to a two-shot of them, Kealoha puts his gun away all over again. (00:32:30)

Hawaii Five-O (1968)

Pray Love Remember, Pray Love Remember - S1-E14

Continuity mistake: Danno walks several feet into McGarrett's office to make a report. Though there's no break in the dialogue and no time for him to have moved, the shot cuts and he's suddenly standing outside the door as they finish their conversation. (00:02:45)

Trivia: Kato was Japanese when The Green Hornet premiered as a radio series in 1936. After the Japanese attack on Pearl Harbor in 1941, Kato suddenly became a Korean. In the 1960s, when future martial arts superstar Bruce Lee (who was Chinese) assumed the role for television, Kato was meant to regain his Japanese heritage. But his ethnicity was never mentioned on the show, and publicity simply referred to Kato as "Asian."

Jean G

The Scorpio Drop - S3-E14

Continuity mistake: Angela's limo picks up Mundy and drives down a tree-lined country road. But in the very next shot, the view out the back window is of a busy city street with nary a tree in sight. (00:13:00)

Jean G

To Lure a Man - S3-E13

Factual error: Mundy makes a phone call by wiring a radio speaker to the wall jack. Clever, but it wouldn't work. He might be able to hear dial tone, etc., but no one he called would be able to hear him without a microphone/sound pick-up, which the radio didn't have. (00:16:15)

Monk (2002)

Mr. Monk Is At Your Service - S5-E12

Continuity mistake: Monk finds the magazine from which the butler cut out words for his blackmail note, and matches a scrap with the word "Police" on it to the article it was cut from - only the left and bottom edges of the scrap don't conform at all to the hole cut in the page. They haven't been trimmed more. In fact, the whole piece is much too large to fit into the space. (00:36:30)
