Monk (2002)

Mr. Monk Goes to Jail - S2-E16

Continuity mistake: A fight breaks out in the prison laundry, and Monk crawls into a giant dryer to hide, pulling the door mostly shut after him. But in the very next shot of the fight raging on, the dryer door is visible in the background - wide open. Next shot after that, it's shut again, and Monk sneaks it open to peek out. (00:42:00)

Quantum Leap (1989)

Nuclear Family - October 26, 1962 - S3-E21

Audio problem: Just before the dog carries in the sack of shotgun shells, Mack and Sam have an argument, and Mack's dialogue is very poorly looped. Throughout the scene, the sound is out of sync with his lip movements. (00:33:00)

Batman (1966)

The Dead Ringers (2) - S2-E16

Continuity mistake: Batman folds up the Bat Shield and starts to tuck it into his belt, which it is much too large to fit into. In the very next shot, the shield has disappeared. (00:21:00)

Batman (1966)

The Dead Ringers (2) - S2-E16

Continuity mistake: Tied to a conveyor belt, Batman and Robin move past a stack of boxes stenciled with the factory's name. Cut to close-up, and they're now back to a position right next to the boxes. Back to master shot, and the boxes are once again several feet away - at least until the next close-up. (00:02:50)

Batman (1966)

The Yegg Foes in Gotham (2) - S2-E14

Continuity mistake: Just before Bruce Wayne tosses the grape to set it off, Egghead's miniature radar bomb moves to a different position on the floor all by itself. (00:04:00 - 00:05:20)

Monk (2002)

Mr. Monk Gets Jury Duty - S4-E16

Continuity mistake: In the jury room, the two windows behind Monk have venetian blinds, which are raised halfway on the right window. But in several shots of the science teacher coming over to identify the white powder, the right window's blinds are pulled down. (00:35:00)

Torchwood (2006)

Something Borrowed - S2-E9

Factual error: Jack calls Gwen, and when Rhys takes the phone and hangs up on him, Jack's cell phone, for the second time in the series, sounds an audible, impossible dial tone. (00:13:00)

Torchwood (2006)

From Out Of The Rain - S2-E10

Trivia: For this episode, writer Peter J. Hammond borrowed his own idea - from a story he first used in the 1979 Sapphire & Steel series. There, ghosts from old photographs came to life and could trap the living inside the pictures. Here, very similar mischief is afoot, with the ghosts coming out of old silent films instead.

Monk (2002)

Mr. Monk Bumps His Head - S4-E11

Factual error: It's late at night when Roger is stung by hundreds of bees at the lookout point. But bees (both the European and Africanized varieties) are not active in late night hours. They're diurnal (active only during the day). Unless something breaks into their hive, which doesn't happen here, they don't fly or swarm at night. (00:42:00)

Suggested correction: Maybe it is early in the morning and did he not drop the waitress's body down lookout point? Also the body could very well have triggered the bees and the hornets.

Quantum Leap (1989)

Last Dance Before An Execution - May 12, 1971 - S3-E19

Continuity mistake: When Sam is strapped into the electric chair, the huge clock on the wall behind him, which is there so that both the prison doctor and the reporters viewing the execution can mark the time of death, reads 7:00. The final scene runs for more than five minutes - after which the clock still reads 7:00. (00:37:15 - 00:42:00)

Last Dance Before An Execution - May 12, 1971 - S3-E19

Continuity mistake: As Sam is marched down the corridor to the electric chair, Raul reaches through the cell bars and begs him to confess. In long shots, Raul's arms are fully extended. But in close-ups, his arms are folded and wrapped around the bars. (00:38:30)

The Clock King Gets Crowned (2) - S2-E12

Continuity mistake: Batman tosses a large white box at one of the henchmen. The box lands on the round dais of the clockworks, but as that dais begins to rotate, the box disappears. It pops back up a few shots later, now sitting on the stationary corner of the clockwork platform. But as the fight rages on, it vanishes again between shots. (00:21:30 - 00:22:10)

Batman (1966)

The Clock King's Crazy Crimes (1) - S2-E11

Continuity mistake: During the fight in the art gallery, the papers on the desk to the left of the screen are scattered. But in some shots of the fight progressing, the same papers have straightened themselves back into nice, neat stacks again. (00:17:00)

Revealing mistake: The giant crystal's red beam strikes Zaran at the end, turning him into a skeleton. But the lab specimen used for this effect is a little too obviously just that: you can see the pins holding the bones together, as well as the surgical saw incision in the skull. (01:27:00)
