Monk (2002)

Mr. Monk Makes the Playoffs - S7-E13

Continuity mistake: Just where was that evidence locker? Randy borrows a big screen TV from the evidence room and pushes it upstairs toward the ground floor office, indicating the evidence locker is in the basement. But when Natalie arrives and walks through the office, she goes up the stairs instead of down to find Randy with the TV wedged in the stairwell, indicating the evidence room is on the upper floor. The TV wedged itself tight on his way up the stairs, so he hasn't moved it in the meantime: he's in exactly the same spot with the same picture on the wall. (00:40:30)

Duel (1971)

Visible crew/equipment: When Mann's bumper gets caught under the school bus and he's trying to free it, a large lighting panel is reflected in his car door every time he opens it. (00:47:00)

Duel (1971)

Revealing mistake: At the reptile farm, the phone booth Mann uses to try and call for help has no phone lines attached to it, above or below ground (you can see that there are no underground wires when the truck mows the booth down). It's definitely not a wireless/radio telephone, either. There should be lines running to it, but there are none. (00:57:00)

Duel (1971)

Continuity mistake: Mann's '71 Plymouth Valiant has the ignition slot in the steering column. But in one insert shot of his speedometer as he races the truck at 100 mph, an older model's dashboard ignition slot is visible - with no key in it. (01:10:00)

Forever Knight (1992)

I Will Repay - S1-E10

Character mistake: Nick asks Janette if she's ever turned anyone else into a vampire - an odd question, considering the fact that he already knows she has. He was there when she brought the baroness across in the 18th Century flashback sequence for "If Looks Could Kill." It's even odder that Janette tells him she's never done it, since she knows that Nick knows she has. Apparently, the only one not in the know here was the scriptwriter.

Duel (1971)

Visible crew/equipment: When Mann's car spins out on the gravel and comes to rest facing the truck, you can briefly glimpse the cameraman (purported to be Steven Spielberg himself) crouching in the back seat. (01:04:25)

Monk (2002)

Mr. Monk and the Magician - S7-E15

Continuity mistake: "Mr. Monk & the Magician": While Monk and Natalie talk to the magician's assistant, the white rabbits in the hutch shift positions instantly with each change of camera angle. (00:20:00)

Monk (2002)

Mr. Monk and the Magician - S7-E15

Continuity mistake: In the basement, the configuration of the ropes tying Monk to the chair changes repeatedly between shots, from overlapped to straight across and back again. (00:42:00)

'V' For Vashon: The Father (2) - S5-E10

Character mistake: Hayward tells Vashon that McGarrett's office is on the third floor of the Iolani Palace. Previous episodes have placed the office on the second floor, or have shown the palace to have only two floors. (00:31:45)

Revealing mistake: When Van Helsing arrives at Castle Dracula and discovers the Count's bride lying staked in her coffin, the "corpse" is visibly breathing. (00:29:10)

Continuity mistake: When Charlotte hastily makes the bed and shoves Ted out onto the window ledge, the bed covers rearrange themselves between shots: a purple sheet that she'd tucked in before is now peeking out over the bedspread. (00:15:05)

Trivia: More than a dozen writers worked on various versions of the Oz script. Some of their "original" ideas that were (thankfully) scrapped: an opera-singing Princess Betty of Oz; a stupid son for the Wicked Witch with ambitions to be King of Oz; and a budding romance between Dorothy and one of the farmhands. For a while, the Wicked Witch was to be glamorous (a la Disney's witch in Snow White), but fortunately, saner heads prevailed and kept her as L. Frank Baum had originally written her.

Trivia: During filming, Toto was accidentally stepped on by one of the Witch's guards, and had to be replaced for several days with a look-alike. Other on-set accidents included two winged monkey actors who fell when their support wires snapped, and Margaret Hamilton being severely burned when the elevated platform that made her disappear from Munchkinland in a puff of smoke malfunctioned.

Hawaii Five-O (1968)

Journey out of Limbo - S5-E8

Continuity mistake: When the police cruiser with Danno aboard leaves HPD HQ, it's a 1967 Ford Galaxy 500 with wind wings. But in the next shot of it on the Honolulu streets, the car is a '71 or '72 model with no wind wings. (00:43:30)

Camera Obscura - S2-E36

Factual error: Gingold's camera obscura shows us a cross-fade. That's a special effect the Victorian device wasn't capable of performing. (00:06:45)

The Waiting Room - S2-E51

Continuity mistake: It's the 1800s, and Dicter rides up to a building that has every window, upstairs and down, blazing with what is obviously electric light. When he walks into the ground floor saloon, however, there are no bright lights. It's dark and dim inside. (00:03:00)
