Continuity mistake: "The Cyborg": At the end, Crane shoots the cyborg Nelson in the forehead. It clutches at its completely intact face and falls down. We cut to crew reactions and then back to the fallen cyborg - and now its head is nothing but a jumble of circuits. Were those delayed-action bullets in Crane's gun? (00:46:40)

Continuity mistake: "The Cyborg": To expose the cyborg duplicate of himself, Nelson disrupts the nerve controls on the computer facsimile's left hand. But the cyborg Nelson's right hand is the one that subsequently malfunctions. (00:45:20)

Tiger Left, Tiger Right - S2-E6

Continuity mistake: Kimble's been kidnapped and held for ransom. He had nothing with him when he was taken, yet when he leaves the abandoned house with the kidnappers, he has a small suitcase. When they drop him off on the road, he doesn't have it. When, in the epilogue, he's shown walking, he's carrying a different, larger suitcase. (00:46:00 - 00:49:00)

Continuity mistake: 8-15 "Deadly Persuasion": Danny says he hasn't fired his old 45 automatic "since I got out of the service." But at least two previous episodes had established that he joined Five-O right out of college, and was never (not even before college) in the service.

Continuity mistake: "Robin & the Sorcerer, Part 1": When Robin's men ambush the sheriff's soldiers, two are struck by arrows and fall from their horses. A few shots later, we see them both lying dead on the ground, but the arrows that killed them have disappeared. (00:46:40)

Deliberate mistake: 9-12 "The Bells Toll at Noon": When McGarrett picks up Danny and drives away in his Mercury Grand Marquis, the rear view mirror is missing, probably removed to prevent it from blocking the actors' faces. It reappears later on. (00:33:00)

Death is a Company Policy - S5-E1

Continuity mistake: The Five-O computer here lists McGarrett's name as Steven J. McGarrett. But in "A Gun For McGarrett," his middle name is revealed to be Aloysius. Strange that the computer doesn't know the name of its own department's head.

Factual error: The small round table in West's railcar parlor has a flip-over top, with secret maps hidden on the underside. The wine decanter and glasses all stay attached when it flips. But impossibly, when West turns the tabletop back over at the end, there's a full cup of coffee sitting on it. (00:48:30)

Revealing mistake: When the marshal opens the train's rear door and comes in to arrest Lana, you can see wrinkles in the "sky" outside. It's really a painted cloth backdrop. (00:48:40)

Revealing mistake: Mr. Braine fires a mini-rocket at Brendage, killing him. But a poor camera angle reveals the explosive charge going off in the wall more than two feet to Brendage's left. From the angle at which it was fired, the rocket couldn't possibly have hit him. (00:21:45)

Revealing mistake: Mr. Braine screams as his steam-powered chair crashes into the bandstand. If you look closely at the next shot, just before the explosion, you can see that the dummy now in the chair has its head bowed forward. It also has light brown hair, rather than Mr. Braine's silver hair. (00:47:10)

Factual error: Though this episode takes place on April 1st, 1969, a freshly-painted sign in the park contradicts the date. In the scene with the hippies in which Sam is disguised as a meditating guru, the huge sign just behind him touts the San Diego Centennial Celebration - in 1968. (00:16:00)

Factual error: "Night of the Lord of Limbo": At the end, Vautrain tells West and Gordon to go through the door on their right. The door is on the camera's right, but is on West's and Gordon's left. (00:45:10)

Continuity mistake: West and Gordon are ordered to sit on the floor to talk to the diminutive Dr. Loveless at eye level. In all the two-shots, their shoulders are touching. In all the wider angles, they're sitting a foot apart. (00:09:05)

Continuity mistake: When Gerard leaves the cell block, Kimble is at the cell door with his hands on the bars. Next shot, as Gerard exits, the cell door is visible in the background, and Kimble is no longer standing there. But in an immediate cut back to the cell, he's back with his hands on the bars again. (00:35:35)
