Factual error: After falling into the pit of needles, somehow all of the needles in Amanda's skin stay intact and do not break, despite her fall and her rapid digging through them. Very, very unlikely at best.

Continuity mistake: When Amanda and Daniel entered the bathroom from 'Saw', Adam's body is shown with his right leg shackled. However, in 'Saw' and in Adam's death flashback in 'Saw III', his left leg is shackled.

Trivia: The needle pit scene was done with real syringes. The crew had to replace the tips with fibre optic ones. The pit was filled with slime to make the syringes slide around more. 120,000 syringes were used during this scene.

Answer: It was Zep, but he died because Adam smashed his head with a toilet seat cover.

Chosen answer: It was Zep, the guy from the first film that jigsaw used to kidnap the doctor's wife and kid. He went down to the room at the end of the first film, then got shot, so ended up dead down there too.

Answer: The man was Zep Hindle, an orderly, who died in the first Saw after he was beat to death with a toilet tank lid by Adam.
